With the beggining of Monster, Dr. Kenzo Tenma is a young Japanese brain surgeon, working at Eisler Memorial Hospital in Dusseldorf. A natural-born humanitarian who cares about the lives of others, his kindness influences those he meets. Soon dissatisfied with the political bias in treating patients, he seizes the chance to change things after a massacre brings fraternal twins Johan and Anna Liebert into the hospital, the former whom he revives of a fatal gunshot wound. Nine years later, through Adolf Junkers, Tenma learns the scope of the atrocities committed by Johan, and vows to fix the mistake he made by ending the "Monster's" life himself.

The older of twins to Michael Liebert, an East German trade advisor who took refuge to the West. Johan Liebert is the titular 'monster' and the principal antagonist of the series. He is refered numerously as the next Adolf Hitler and the devil himself. Charming, charismatic and suave at first glance he comes off as aloof to those around him; soon revealing a self only cold, cruel and fundamentally incapable of empathy.

The younger of twins to Michael Liebert and Johan's fraternal sister, Anna Liebert is first introduced as the only physically unharmed survivor in what appeared to be a botched burglary at the Liebert family home. After seemingly disappearing with Johan, she was adopted by the Fortner family and subsequetly renamed to 'Nina.' Having blocked out most of her childhood, she is a bubbly, amiable and sincere young woman. With no more than nightmares and apparent childhood amnesia, Nina lives at peace until Johan contacts her on their 20th birthday having the Fortners killed. Following this, she avidly pursues Johan in search of her past.

Ex-spy and freelance journalist Wolfgang Grimmer reasearching 511 Kinderheim becomes drawn into the search for Johan after an enounter with Tenma on their way to Prauge. Always smiling, he is a friendly, polite man captivated by people and culture. However, as a former subject in 511 Kinderheim he confides in not having ever experienced genuine love or grief, rather in life having created an agressive alter ego whom he dubs the "Magnificent Steiner" as a kind of psychological filter to justify his acts of violence.

Tenma's ex fiancee and the daughter of the Director, Eva Heinemann is an independent, yet superficial woman characterised by her excessive drinking. Several years and three failed marriages later, she decides to once again persue Tenma before being rejected near-instataniously. After being shot down by him a second time, she chooses to trace Tenma as the culprit in her father's murder, obsessively hoping to see him suffer in prison in retaliation for his rejection of her.

Inspector Lunge is a BKA detective assigned to the inital murder case at Eisler Memorial Hospital. With Tenma as his main suspect, Lunge initially thinks Johan was imagined by Tenma as an excuse, but progresses to the belief Tenma suffers a split personality. He seems devoid of emotion, which allows him to commit himself to every case on which he works.

Bashful yet oddly brazen, Dieter is a young boy Tenma encounters in his early search for Johan. When Tenma first meets him, he is a timid orphan under the care of a man named Hartmann; Tenma later discovers that Dieter is physically and psychologically abused by Hartmann who plans to make him into "a second Johan" by applying the same conditioning used in the 511 Kinderheim experiment, but with seemingly no success. After being rescued by Tenma, Dieter tags along in his search for Johan, later residing with Nina and Reichwein.

Having spent most of his life in foster care, Karl is a naive albiet thoughtful student at the university of Munich working in the same anthropology department as Lotte Frank. Adopted by the Neumanns in his later adolesence, he's shown to care for and accept the pair as his 'parents.' Though despite this, he searches for his biological father, ultimately leading him to Hans Schuwald, an influencial buissnessman who takes aid in local university students. Karl tries to get close to Schuwald without letting him know that he is his son. During this time he meets Johan as a fellow university student immediately befriending and confiding in him through their seeming similarities.

Lotte Frank is a student at the University of Munich who trying to win Karl Neumann's heart, helps in his investigation of his father and late mother. Naturally, she's bossy, nosy and a airheaded, but passionate and caring nontheless. When Karl rejects, and unintentionally humiliates her by getting a proxy to go on a date with her, she is crushed and later comforted by Nina whom she meets at the library; they become fast friends.

Jan Suk is a new detective with the Prague police trying to solve the mystery involving the death of his superior detective Zeman. Zeman once investigating the death of a former headmaster at 511 Kinderheim, with Grimmer as a suspect, it was discovered he was was working with the Czechoslovak secret police. When Chief Havrik, Chief Patera and Detective Janacek are mysteriously poisoned with candy given to one of the detectives by Johan Liebert, Jan Suk is under suspicion and his movements monitored. He confides nightly in Johan (who was masquerading as Anna) at a bar.
